Most systems don’t fail because they are poorly designed. They fail because humans gradually lose the ability to change them intentionally. As abstraction accumulates and feedback is delayed, people can still explain the work, but they can no longer steer it with confidence. This paper names that loss Operational Grip, describes how it degrades, and explains why it appears across modern systems—from software and AI to organizational frameworks.
